Supplier Registration Checklist

Please see the checklist below for the items you will need to complete your registration. To register, you will need the following pieces of information at a minimum:

  • General Company Information: including legal Company Name, and Doing Business As (DBA) if applicable
  • Internet Address, Federal Tax I.D., Dun and Bradstreet Number
  • Parent Company Name and Federal Tax I.D.
  • Executive and Primary Contact Information: The Primary Contact will receive Alerts as documentation nears expiration.
  • Business/Financial Information including Year Established, Gross Annual Sales for the last 3 years, number of employees and Legal Structure.
  • Capabilities: Products and Services Information including Specific Commodities AND NAICS Codes (North American Industry Classification Codes), Product/Service Description, and Geographical Service Areas.  The first capability listed in each section will be identified as your “primary” capability. All Vendors should choose the applicable NAICS code(s) and if applicable, the Shaw capability codes(s). If there is not an applicable Shaw Capability code, select “1-Not Applicable” from the Available Items. A “%” sign can be used as a wildcard to search the NAICS and Shaw Capability fields.
  • Diversity Information: Companies may use this tool to indicate that they are a diversity Vendor or contractor (minority-owned business, women-owned business, small business, small disadvantaged business, HUBZone business, veteran owned business, and service-disabled veteran owned business) and follow instructions to attach any certifications and the affidavit.
  • Representations and Certifications: Any company that is either currently performing on a Federal project, or wishes to compete for Federal work, MUST provide Representations and Certifications by completing a copy of the Shaw General Representations and Certifications form (found in the Reps and Certs Survey section). In order to trigger the appropriate survey, please be sure to fill in the boxes for Reps and Certs start and end dates on the “Profile” page. Use the date that you are completing your profile as the start date for the Reps and Certs. All end dates must be entered to end one day prior to the start date (i.e. if start date is 11/11/2011, end date must be 11/10/2011). 
  • Reference Information: A total of three references can be recorded
  • Insurance Certificates/W9

Once you have completed the process, you will receive an e-mail confirming that your registration is complete.
